Болезни системы кровообращения остаются главной причиной смерти в мире. Современные исследования указывают на необходимость выхода за рамки традиционных биологических факторов риска и подчеркивают важность учёта социально-дистрессовых детерминант – социальных условий, вызывающих хронический стресс (экономическое неравенство, условия труда, социальная изоляция), которые оказывают значительное влияние на сердечно-сосудистое здоровье.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: провести комплексный анализ современных глобальных тенденций и региональных стратегий в области изучения, оценки и управления социально-дистрессовыми детерминантами для профилактики болезней системы кровообращения в 2021–2025 гг.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Выполнен систематический анализ научной литературы и официальных документов за 2021–2025 гг. Поиск проводился в базах данных PubMed, MEDLINE, Google Scholar, eLIBRARY.ru, а также на ресурсах Всемирной организации здравоохранения, Европейского общества кардиологов и Американской кардиологической ассоциации. В итоговый анализ вошли источники, включающие оригинальные исследования, систематические обзоры, клинические руководства и аналитические отчёты. Использовались методы сравнительного и контент-анализа.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлено усиление влияния макрокризисов (пандемия COVID‑19, климатические изменения) как супердетерминант, усугубляющих социальный градиент в  исходах болезней системы кровообращения и  вызвавших глобальный рост психологического дистресса. Установлен тренд на интеграцию психического здоровья в кардиологическую помощь (рекомендации Европейского общества кардиологов), показано смещение фокуса профилактики на создание здоровьесберегающей среды через фискальные и градостроительные политики («здоровье во всех политиках»). Проведён сравнительный анализ региональных стратегий управления социально-дистрессовыми детерминантами в Европе (социальное назначение), Северной Америке (стандартизированный скрининг), Азии (борьба с загрязнением воздуха, корпоративные программы) и странах с низким доходом (общинные проекты, программа mhGAP). Освещены методологические аспекты оценки социально-дистрессовых детерминант, включая вопросы валидизации комплексных опросников через проверку конвергентной валидности с международными шкалами (WHOQOL-BREF, PSS, SCORE2).</p></sec><sec><title>Заключение</title><p>Заключение. Период с 2021 по 2025 г. ознаменовался переходом от накопления знаний о вреде социально-дистрессовых детерминант к активному внедрению практических стратегий по их нивелированию. Перспективы связаны с персонализацией профилактики и межсекторальным сотрудничеством. Ключевыми вызовами остаются сопротивление промышленного лобби, сложность оценки долгосрочной эффективности и риски цифрового неравенства. Учёт социального контекста является необходимым условием для снижения глобального бремени болезней системы кровообращения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Relevance</title><p>Relevance. Circulatory system diseases remain the leading cause of death worldwide. Current research points to the need to go beyond traditional biological risk factors and account for socially distress determinants – social conditions that cause chronic stress (economic inequality, working conditions, social exclusion) which have a significant impact on cardiovascular health.</p></sec><sec><title>The purpose of the study</title><p>The purpose of the study: conduct a comprehensive analysis of global trends and regional approaches to the study and management of social-distress determinants for circulatory system diseases prevention in 2021–2025.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A systematic analysis of scientific literature and official documents for 2021–2025 has been carried out. Searches were conducted in PubMed, MEDLINE, Google Scholar, eLIBRARY.ru, and resources from the WHO, the European Society of Cardiologists, and the American Association of Cardiology. The final analysis includes sources including original studies, systematic reviews, clinical guidelines and analytical reports. Comparative and content-analysis methods are applied.</p></sec><sec><title>Results</title><p>Results. The increasing influence of macro crises (COVID‑19 pandemic, climate change) as a super-determinant has been identified, worsening social gradient in BSC outcomes and causing a global rise in psychological distress. Established a trend for integrating mental health into cardiac care (recommendations of the European Society of Cardiologists 2021/2023). The focus of prevention is shifted to creating a health-saving environment through fiscal and urban policies (“health in all policies”). Comparative analysis of regional strategies for managing social-distress determinants in Europe (social purpose), North America (standardized screening), Asia (air pollution control, corporate programs) and low-income countries (community projects, mhGAP programme). Methodological aspects of the assessment of social-distress determinants are highlighted, including questions of validation of complex surveys through verification of convergent validity with international scales (WHOQOL-BREF, PSS, SCORE2).</p></sec><sec><title>Conclusion</title><p>Conclusion. Period 2021–2025 was marked by a shift from accumulating knowledge about the harm of social-distress determinants to actively implementing practical strategies for their nivelling. The prospects are linked to the personalization of prevention and intersectoral cooperation. Key challenges remain industrial lobby resistance, the difficulty of assessing long-term efficiency, and the risks of digital inequality.
